Amcare Pro Home Health, Palliative Care & Hospice is seeking compassionate LVNs for PRN field positions serving customers in Melissa, Blue Ridge and surrounding areas. You will provide skilled nursing visits in patients’ homes and support independent living and quality of life.

Responsibilities include administering medications, documenting care, and communicating with the care team. Travel across Fort Worth, Arlington, and nearby areas is required, with flexible scheduling for PRN staff.

Qualifications

• Active and unrestricted Texas LVN license required.
• Current CPR or BLS certification.
• At least one year of nursing experience preferred.
• Home health, hospice, or palliative care experience preferred.
• WellSky/Kinnser experience is a plus.

Responsibilities

• Perform skilled nursing visits in patients’ homes.
• Provide nursing care according to physician orders and the established plan of care.
• Administer medications and treatments within the LVN scope of practice.
• Monitor patients’ conditions and promptly report significant changes to the supervising RN or physician.
• Educate patients and caregivers regarding medications, treatments, safety, and care instructions.
• Communicate regularly with the supervising RN and interdisciplinary care team.
• Complete accurate and timely clinical documentation.
• Follow infection-control procedures, agency policies, and applicable regulations.
• Travel to patients throughout Fort Worth, Arlington, and surrounding areas.
