Job summary
Amcare Pro Home Health, Palliative Care & Hospice is seeking compassionate Licensed Vocational Nurses for PRN field positions serving patients in Melissa and surrounding areas. You will perform skilled visits, administer medications, and educate patients and caregivers in their homes.

Qualifications
- Active Texas LVN license.
- Current CPR or BLS certification.
- At least one year of nursing experience preferred.
- Home health, hospice, or palliative care experience preferred.
- WellSky/Kinnser experience is a plus.
Responsibilities
- Perform skilled nursing visits in patients’ homes
- Provide nursing care according to physician orders and the plan of care
- Administer medications and treatments within the LVN scope of practice
- Monitor patients’ conditions and report significant changes to the RN or physician
- Educate patients and caregivers on medications, treatments, safety, and care instructions
- Communicate with the RN and care team
- Complete accurate and timely clinical documentation
- Follow infection-control procedures and policies
- Travel to patients throughout service area
