Job summary

Lifepoint Rehabilitation is seeking a Case Manager (CM), Registered Nurse (RN) to support discharge planning, care coordination and patient advocacy across the care team. The role emphasizes timely IPoC documentation, interdisciplinary collaboration and effective communication with families and payers.

Responsibilities include coordinating care conferences, assisting with performance improvement, and ensuring comprehensive discharge plans with home health and equipment as needed.

Qualifications

• Must hold a current professional license as a Registered Nurse (RN) or equivalent.
• Experience in case management, discharge planning and care coordination is preferred.
• Ability to collaborate with physicians, nursing, therapy and other departments.

Responsibilities

• Completes departmental orientation, initial and annual competencies.
• Assists with performance improvement initiatives collecting and reporting data.
• Consults with other staff to collaborate in patient care delivery and discharge planning.
• Completes IPoC documentation per CMS guidelines.
• Schedules conferences and communicates progress to patient and caregiver.
• Coordinates weekly care team conferences to refine treatment plans.
• Coordinates discharge needs including home health, equipment and services.
• Performs intake assessment within 24–72 hours of admission and follow-ups as policy.
• Participates in utilization review activities and communicates updates to payers.
