Job summary

Eden Senior Care invites a PRN Certified Occupational Therapist Assistant (COTA) to join Evansville Manor, a skilled nursing facility within our network. You will provide rehabilitation services under the supervision of a licensed Occupational Therapist at one or more service locations.

The role emphasizes implementing treatment plans, direct resident therapy, progress reporting, and collaboration with the multidisciplinary team in a supportive environment.

Qualifications

• Wisconsin COTA license in good standing
• Experience in skilled nursing preferred but not required
• Ability to follow individualized treatment plans and collaborate with therapists

Responsibilities

• Assist in implementing individualized treatment plans developed by the Occupational Therapist
• Provide direct therapy services to residents under supervision
• Communicate resident progress or concerns to the OT and interdisciplinary team
• Instruct families and nursing staff on follow-through programs
• Maintain documentation of treatment and progress per guidelines
• Ensure therapy equipment is safe and the workspace is clean
• Follow facility protocols and safety standards for residents and staff
