Job summary

PACS Group is seeking a Certified Occupational Therapy Assistant (COTA) to join Kern River Transitional Care. The role requires delivering resident-centered therapy under the supervision of an Occupational Therapist in a skilled nursing setting.

You will support residents in regaining independence and improving daily living activities. The ideal candidate holds an OTA degree, an active CA license, CPR if required, and prior SNF rehab experience.

Qualifications

• Must have an associate degree from an accredited OTA program.
• Current California OTA license in good standing.
• License must remain active and meet CE requirements.
• CPR certification if required by licensure or facility policy.
• Experience in skilled nursing or rehabilitation preferred.
• Experience with Net Health or PointClickCare preferred.

Responsibilities

• Provide OT treatments as directed by supervising OT.
• Follow care plans to improve ADLs and independence.
• Document accurately and timely per policy and regulations.
• Communicate resident progress with supervising OT.
• Collaborate with physicians, nursing, and rehab team on discharge planning.
• Educate residents and families on treatment goals and safety.
