Job summary

Qualifications

• Demonstrated ability to utilize independent and effective clinical judgment, problem-solving and decision making skills.
• Meet physical requirements to deliver responsibilities.
• Experience in a geriatric rehabilitation setting preferred.
• Successful completion of hiring process / background check.

Responsibilities

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Develop an overall plan of care to include treatment, goals and discharge plan; set duration and frequency.
• Perform skilled treatments per established plan of care.
• Document observations and patient progress within required timeframes.
• Complete all billing and documentation on the date of service.
• Maintain rehab gym equipment and treatment area with infection prevention in mind.
