Job summary

Care Guide Partners PACE, part of Hosparus Health, seeks a PRN Certified Nursing Assistant (CNA) to deliver personal care and related services to PACE participants under the IDT, in clinic or home settings as needed. The role requires KY SRNA certification and CPR, with preferred experience in home health or nursing assistant work.

Applicants should have a valid driver’s license with proof of auto insurance and familiarity with EMR systems.

Qualifications

• High school diploma preferred.
• SRNA certification required in Kentucky.
• Minimum 1 year home health aide or nursing assistant experience preferred.
• Must have a valid driver’s license with proof of automobile insurance.
• CPR certification required.
• Knowledge of safety and infection control in healthcare facilities.
• Ability to communicate clearly and effectively.

Responsibilities

• Deliver personal care and related services to PACE participants.
• Collaborate with Interdisciplinary Team (IDT) and follow plan of care.
• Work primarily in clinic/day center; may visit homes, long-term care facilities, or hospitals.

Education

State Registered Nurse Aide (SRNA) certification

Home health aide experience

CPR certification

Valid driver’s license

Experience with elderly/frailty

Electronic medical records
