Continuum Therapy Partners, LLC in Melrose, MN is seeking a Certified Occupational Therapy Assistant to join our therapist-led team on a PRN basis. The role supports individualized treatment plans and patient progress in collaboration with an Occupational Therapist.

You will educate patients on adaptive equipment and home exercise programs, documenting changes and maintaining clear communications with the supervising OT.

Qualifications

• Associate Degree in Occupational Therapy Assistant required.
• Current, unencumbered state COTA License; NBCOT Certification required.
• 1 year of previous experience as a Certified Occupational Therapy Assistant preferred.
• Implements individualized treatment plans under the supervision of an Occupational Therapist.
• Documents patient progress and reports changes in patient status to the supervising OT.
• Educates patients on adaptive equipment and home exercise programs.
