Job summary

Unity-Hospice is seeking a Certified Nursing Assistant (CNA) for Meng Residence in De Pere to provide patient care under the supervision of an RN. CNAs will cross-train to support community visits and may handle a small caseload as a float CNA when needed.

The role is full-time, 36 hours per week, with three 12-hour shifts from 6:00 a.m. to 6:30 p.m., plus every other weekend, on-call shifts, and 2–3 holidays per year. Benefits are provided in the Unity package as described in the posting.

Qualifications

  • Completion of certified nursing assistant program.
  • Must be listed on the Wisconsin Nursing Assistant Registry and eligible to work at state and federally certified facilities.
  • Valid driver's license, auto insurance, and access to a reliable vehicle.

Responsibilities

  • Assist with activities of daily living (ADLs).
  • Provide restorative care and skin care.
  • Support mobility, nutrition, and hydration needs.
  • Monitor vital signs and report changes to the RN.
  • Document care per the plan of care and CADD pump readings.

The primary role of the Certified Nursing Assistant (CNA) is to provide care and support to patients at Meng Residence in De Pere. The CNA provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN's comprehensive assessment of the patient's personal care needs and are documented in the Activities of Daily Living Plan of Care.

As part of this role, CNAs will be cross-trained to provide support within the community as needed to see patients. During times when census or staffing needs require additional support, you may assist the community team by managing a small patient caseload, working as a float CNA, or providing care to patients in their homes or other facilities. This cross-training provides flexibility and helps ensure consistent, high-quality care across all settings as patient and organizational needs fluctuate.

Schedule: Full-time, 36 hours per week, working three 12-hour shifts from 6:00 a.m. to 6:30 p.m. Includes every other weekend (with occasional fill-in shifts as needed), participation in a 10- 15-shift on-call rotation, and 2-3 holidays per year.
