Job summary

Select Specialty Hospital is seeking a Case Manager (PRN) to support discharge planning, utilization reviews, and care coordination at our Critical Illness Recovery Hospital. The role involves working with a multidisciplinary team to ensure safe, timely patient flow.

The ideal candidate holds a current license in nursing, respiratory therapy, or social work per state guidelines; prior discharge planning in an inpatient setting is preferred.

Qualifications

• Requires licensure in a clinical discipline (RN/LPN/LV?N) or RT or Social Work SW/MSW as per state guidelines.
• Previous inpatient discharge planning experience is preferred.
• Must be able to work a minimum of 2 shifts per month on a dayshift schedule (weekdays).

Responsibilities

• Develops and maintains discharge plans that are patient-specific, timely and safe.
• Performs utilization criteria reviews.
• Coordinates with payors to optimize reimbursement and resource management within the plan of care.
• Completes medical record documentation and reports to the Director of Case Management.
• Leads multi-disciplinary team meetings and ensures compliance with Utilization Management policies.
