Job summary

Select Specialty Hospital, a Select Medical facility, is seeking a PRN Case Manager to support post-ICU patients in a LTACH setting. The role emphasizes discharge planning, utilization reviews, and social work services, with a focus on compliant, patient-centered care.

Qualified candidates will hold active licensure in a clinical discipline or social work per state guidelines and have prior discharge planning experience to coordinate care and ensure appropriate reimbursement.

Qualifications

• Current licensure in a clinical discipline either as a Nurse (RN /LPN/ LVN) or a Respiratory Therapist OR Social Work SW/MSW (per state guidelines)

Responsibilities

• Develops and implements a patient-specific discharge plan.
• Verifies utilization criteria reviews.
• Coordinates with payors to assure proper reimbursement and manage costs.
• Facilitates multidisciplinary team meetings.
• Maintains documentation in medical records.
• Reports to Director of Case Management and provides social work services as needed.
