Job summary

Select Specialty Hospital, part of Select Medical, seeks a PRN Case Manager to support discharge planning, utilization reviews, and social work services. You will coordinate with physicians, nurses, therapists and payors, maintain timely documentation, and report to the Director of Case Management.

Experience in inpatient care and chronic critical illness is preferred. Shifts are on an as-needed basis, typically weekdays, with 8 am–5 pm hours; current licensure in nursing, respiratory therapy,

Qualifications

• Current licensure in a clinical discipline either as a Nurse (RN/LPN/LVN) or a Respiratory Therapist OR a current Social Work license per state guidelines.
• Previous RN/LPN/RT/SW/CM inpatient hospital experience in critical care/acute care.
• Adequate experience in an acute medical case management setting and ability to manage a plan of care for chronically critically ill populations.

Responsibilities

• Develops patient-specific, safe and timely discharge plans and coordinates with care teams.
• Performs utilization reviews and coordinates with payors to assure appropriate reimbursement and resource management.
• Maintains documentation in medical records and reports to the Director of Case Management; provides social work services as needed.
