Job summary

Qualifications

• Bachelors degree required; Master's degree preferred.
• Minimum of 2 years of experience in case management related work.
• Valid Fingerprint Clearance Card.
• Completion of BLS and First Aid Training within 30 days of employment.
• Completion of Verbal De-Escalation Training within 30 days of employment.

Responsibilities

• Coordinate and facilitate the effective delivery of quality services to Calvary patients.
• Maintain treatment plans, utilization review, and discharge planning.
• Lead psychoeducation groups and case management activities.
