Job summary

Care Planning Institute, Inc. seeks an RN or LPN to perform in-home assessments for elderly clients in the Dayton, OH area. Assessments are conducted in clients' homes, about 30 minutes, with travel time paid at $40/hour. Flexible schedule from 8am to 8pm, Monday through Sunday; weekends possible.

A TB test and background check are required. Start date soon. As we expand under a VA contract, assignments will be issued as needs arise, with a requirement to complete assessments within 7 calendar

Qualifications

• Licensed registered nurse or licensed practical nurse.
• Experience with in-home health assessments preferred.

Responsibilities

• Conduct in-person assessments for elderly clients in their homes.
• Ask questions about non-medical care needs (bathing, dressing, grooming, light housekeeping).
• Travel to client homes; travel time paid at $40/hour.
• Complete each assessment within 7 calendar days of notification.
• Coordinate with clients and families as needed.
