Job summary

Family Tree Private Care in Colorado is seeking licensed practical nurses (LPNs) for in-home care. We offer full-time, part-time and PRN positions with flexible schedules in a warm, supportive environment that values compassion, family-oriented care, and ongoing professional growth.

As an LPN on our team, you will travel to patients' homes, assess conditions, manage care plans with physician guidance, monitor medications and vitals, and educate caregivers and families on aftercare.

Qualifications

• Active license and certification
• Active CPR/BLS Certification
• A valid driver’s license and reliable transport
• Minimum of 1 year of experience working in the home care industry
• 1 Year of Practicing Nursing

Responsibilities

• Travel to assigned patient’s homes and manage their care plan according to physician’s instructions
• Assess and chart observations of the patient’s condition at each visit
• Complete evaluation tasks, including reviewing medication and vital signs
• Dress or redress wounds and assess the healing progress
• Educating caregivers and family on the aftercare or ongoing care of the patient
• Keep the patient comfortable through palliative care
• Recommend tools or devices that may improve the quality of life for the patient

Pay Rate:

• RN: $48-$55
• LPN: $38- $40
