Job summary

Family Tree Private Care is seeking compassionate Licensed Vocational Nurses (LVNs) to provide in-home care to older adults. Positions are available full-time, part-time, or PRN, with flexible schedule options and a warm, positive, and supportive work environment.

Travel to patients’ homes to implement and monitor care plans, assess vitals, wounds, and medications while educating families. A valid LVN license, driver’s license, and at least 1 year of home-care experience are required.

Qualifications

• Current LVN license and certification
• Minimum 1 year of home-care or related industry experience
• Valid driver’s license with reliable transportation
• Strong patient-centered care and communication skills

Responsibilities

• Travel to patients’ homes and manage care plans per physician instructions
• Assess and chart observations at each visit

• Dress or redress wounds and monitor healing progress
• Educate caregivers and families on post-care

Job Description

We are seeking compassionate Licensed Vocational Nurses (LVNs) to provide in‑home care to older adults. Positions are available full‑time, part‑time, or PRN, with flexible schedule options and a warm, positive, and supportive work environment.

Responsibilities

• Travel to assigned patient’s homes and manage their care plan according to physician’s instructions.
• Assess and chart observations of the patient’s condition at each visit.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Dress or redress wounds and assess the healing progress.
• Educate caregivers and family on the after‑care or ongoing care of the patient.
• Keep the patient comfortable through palliative care.
• Recommend tools or devices that may improve the quality of life for the patient.

Requirements & Qualifications

• Relevant license and certification.

• Minimum of 1 year of experience working in the home‑care industry (or related industry—hospice, home health, hospital, etc.).
