Job summary

Maxim Healthcare Services, Inc. is seeking Registered Nurses (RNs) to join our Transition Team, providing in-home nursing care during a provisional period after patient discharge. Roles are full-time or part-time with competitive hourly pay and weekly payroll.

The Transition Team delivers care across authorized home hours, trains with permanent staff, and supports patients as they transition home before permanent caregivers are identified.

Qualifications

• Current Registered Nurse license in the state of practice.
• Current Health Certificate as required by contract or state regulation.
• Current PPD or Chest X-Ray clearance.
• Current BLS certification.
• Ability to work with both pediatric and adult patients; willingness to travel 45 minutes to 1 hour.
• One year of RN experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Initiate and update plans of care; request provider revisions as patient needs change.
• Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals.
• Respond to changes in patient condition and report to physicians or supervisors as appropriate.
• Administer prescribed medications and treatments and document patient response.
• Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning.

About the Transition Team

Our Transition Team is a supplemental care unit of highly skilled nurses whose primary focus is toquickly and efficiently support patients transitioning homewhile permanent nursing staff are actively recruited. This team is deployed immediately after a patient referral is received and the patient is admitted to Maxim.

Transition Team nurses provide nursing care forall authorized hoursin the home during aprovisional period of 1–90 days. During this time, our office team recruits permanent caregivers. Once identified, those caregivers are given the opportunity totrain and shadow alongside Transition Team nursesin the home before assuming independent care. After the transition is complete, Transition Team nurses move on to support the next new admission—providing variety, consistency, and meaningful impact in patient care.

Requirements

• Ability to work with pediatrics and adults
• Ability to care for (training available) patients with varied acuity (I.e. g-tube, trach, vent)
• One year of RN experience preferred
