Job summary

Maxim Healthcare Services, Inc. is seeking Registered Nurses to join the Transition Team, supporting patients at home during provisional periods. You will work full-time or part-time with flexible scheduling and travel to patient homes within the region.

Qualifications

• Current RN license in the state of practice.
• Current Health Certificate per contract or state regulation.
• Current PPD or Chest X-Ray.
• Current BLS certification.
• One year of RN experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Initiate and update plans of care; request provider revisions as needed.
• Provide therapeutic, preventive, and rehabilitative nursing interventions.
• Respond to changes in patient condition and report to physicians or supervisors.
• Administer prescribed medications and treatments and document responses.
• Identify opportunities for patient/family education, care coordination, discharge planning.

Registered Nurse (RN) – Transition Team

About the Transition Team

Our Transition Team is a supplemental care unit of highly skilled nurses whose primary focus is toquickly and efficiently support patients transitioning homewhile permanent nursing staff are actively recruited. This team is deployed immediately after a patient referral is received and the patient is admitted to Maxim.

Transition Team nurses provide nursing care forall authorized hoursin the home during aprovisional period of 1–90 days. During this time, our office team recruits permanent caregivers. Once identified, those caregivers are given the opportunity totrain and shadow alongside Transition Team nursesin the home before assuming independent care. After the transition is complete, Transition Team nurses move on to support the next new admission—providing variety, consistency, and meaningful impact in patient care.

Responsibilities

Requirements

• CurrentRegisteredNurse (RN) licensein the state of practice
• Current Health Certificate (per contract or state regulation)
• CurrentPPD or Chest X-Ray
• CurrentBLS certification
• Ability to travel 45 minutes to an hour
• Ability to work with pediatrics and adults
• Ability to care for (training available) patients with varied acuity (I.e. g-tube, trach, vent)
• One year of RN experience preferred
