Job summary

Maxim Healthcare Services, Inc. is hiring a Registered Nurse (RN) to support our Transition team in North Carolina. The Transition Team provides care at home during provisional periods to move patients toward home care and support staff recruitment.

Responsibilities include using the nursing process, planning and evaluating care, administering medications, and coordinating discharge planning with supervisors and prescribers. Flexible PT/FT schedules and weekly pay are highlighted in the benefits.

Qualifications

• Current Registered Nurse (RN) license for the state in which the nurse practices.
• Current Health Certificate (per facility RN contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior RN experience preferred.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient care and/or services for contracted clients.
• Initiates the plan of care and requests revisions as patient status changes.
• Performs therapeutic, preventive, and rehabilitative nursing actions based on diagnosis and goals.
• Intervenes in response to changes in patient condition and reports changes to physician/supervisor.
• Administers medications and treatments, and documents effectiveness of interventions.
• Reviews plan of care and program protocols and identifies education, care coordination, and discharge planning needs.

Hourly Pay: $35 - $37 / Hour

Maxim Healthcare is hiring for a Registered Nurse (RN) to work on our Transition team. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim.

Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-90 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then transitioned to another new admission.

Responsibilities:

• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention

Requirements:

• Current Registered Nurse (RN) License for the state in which the nurse practices.


• Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).

• Current BLS card.


• One year prior Registered Nurse (RN) experience preferred.
