Alvita Care is seeking experienced Licensed Practical Nurses (LPNs) for per diem private duty nursing in Connecticut (Fairfield, New Haven, Litchfield County). Shifts range from 6 to 12 hours with day and night options, and you can work part-time or around your current schedule.

Ideal candidates are CT-licensed, with BLS/ACLS/CPR, and 6 months to 1 year of RN experience. Travel to patients' homes is required; drivers are preferred.

Qualificações

• CT LPN licensure required.
• BLS/ACLS/CPR certification required.
• 6 months to 1 year RN experience.
• Ability to explain physician's orders clearly.
• Strong organizational and multitasking abilities.
• Experience with diverse patient populations.
• Knowledge of medical terminology.
• Team player, able to work independently.
• Valid driver’s license or non-driver ID.
• Drivers preferred for travel.

Responsabilidades

• Perform 1:1 patient assessments and plan of care.
• Coordinate with management on complex cases.
• Ensure care follows the plan of care and is properly documented.
• Monitor patient condition and adjust care as needed.
• Supervise LPNs and paraprofessionals providing care.
• Assist with patient discharge planning and communication.
