Job summary

First Day Homecare is seeking an RN or LPN to perform skilled nursing functions in home settings under medical practitioner orders. Per Diem scheduling varies by client assignment, with opportunities for growth in a pediatric private duty nursing context.

The role reports to Clinical Supervisor and involves collaboration with the client’s care team, maintaining documentation, and delivering high standards of care in compliance with regulations.

Qualifications

• Graduate of an accredited nursing program.
• Current RN or LPN license in NJ.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured as required.

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care per medical practitioner orders and client care plan.
• Maintains and documents changes in orders in the medical record.
• Scope includes personal care, monitoring, medication administration, patient teaching, emergency intervention, and pediatric specialized services.
• Collaborates with the client’s care team.
• Ensures safety and a safe environment for clients and coworkers.
• Adheres to Agency policies, procedures, and payor/accreditation standards.
• Participates in care coordination and agency meetings.
• Performs other related duties as needed.

Job Summary

Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of professional standards and the Nurse Practice Act. All nursing services provided will be under the direction of medical practitioner (MD, DO, NP, PA, etc.) orders.

Reports To: Clinical Supervisor
Provides Supervision To: NA – No supervision responsibilities

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client’s care plan.
• Maintains orders for clients and documents all changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client’s care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
• Performs other related duties and responsibilities as deemed necessary.

• Current license to practice as an RN or LPN in the state of operation.
• Current CPR certification.
• Demonstrated written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements.
