Job summary

First Day Homecare is seeking a qualified registered nurse (RN) or licensed practical nurse (LPN) to perform skilled nursing functions under medical orders, in patients' homes. Reports To: Clinical Supervisor. Provides supervision to: NA – No supervision responsibilities.

The role includes high-level nursing care, detailed documentation, and collaboration with the client’s care team to ensure safe, compliant service delivery.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN or LPN license in the state of operation.
• Current CPR certification.
• Strong written, verbal, and interpersonal communication skills.
• Licensed driver with automobile insured in accordance with state requirements.

Responsibilities

• Provide professional nursing care per Nurse Practice Act and client care plan.
• Follow orders and document changes in the medical record.
• Maintain client orders and document changes per agency procedures.
• Provide personal care, monitoring, and medication administration; client teaching.
• Collaborate with care team and promote safety for clients and coworkers.
• Participate in coordination of care and attend agency meetings.
• Perform other related duties as deemed necessary.

• Twice a week wound care and ostomy management

Job Summary

Performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of professional standards and the Nurse Practice Act. All nursing services provided will be under the direction of medical practitioner (MD, DO, NP, PA, etc.) orders.

Reports To: Clinical Supervisor

Provides Supervision To: NA – No supervision responsibilities

Responsibilities

• Provides professional nursing care as defined in the state Nurse Practice Act.
• Provides care in accordance with medical practitioner orders and the client’s care plan.
• Maintains orders for clients and documents all changes in orders in the medical record following Agency procedures.
• The scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration, client teaching, emergency intervention, and specialized pediatric services such as tracheostomy care, ventilator management, and tube feedings.
• Works collaboratively with other members of the client’s care team.
• Promotes personal safety and a safe environment for clients and coworkers.
• Performs job in compliance with Agency policies, procedures, and applicable payor and accreditation standards.
• Participates in the coordination of care and attends Agency meetings as requested.
• Performs other related duties and responsibilities as deemed necessary.
