Job summary

Always Compassionate Home Care in New York is seeking a compassionate LPN or RN nurse for flexible, home-based care. We offer 35$/hour and a 1:1 patient care model in clients' homes, with CPR/BLS required and NYS license.

You will perform care per care plans, educate patients, administer meds, coordinate with other health professionals, and document nursing notes daily. Flexible shifts, supportive team, and immediate openings help you start making a meaningful impact today.

Qualifications

• Active LPN or RN license issued by NYS.
• Minimum of 6 months experience in a home care setting.
• Current CPR/BLS certification.
• Empathy, reliability, and commitment to high‑quality care.
• Ability to perform 1:1 patient care and follow care plans.

Responsibilities

• Perform 1:1 patient care duties and treatments as per care plan.
• Provide health promotion, counseling, and education.
• Administer medications and other personalized interventions.
• Coordinate care with a wide array of health care professionals.
• Document nursing notes regarding patient care daily.
• Must be able to lift 50lbs and stand for extended periods.
