Job summary

Always Compassionate Health in New York State is looking for compassionate LPNs or RNs to join our home-care team in Melville. You will provide one-to-one patient care, health promotion, counseling, and education, and coordinate with a broad network of healthcare professionals.

We offer flexible shifts, competitive pay at $35/hour for LPNs, and immediate openings to start making a meaningful impact in patients' lives today.

Qualifications

• Active NYS LPN or RN license.
• Minimum of 6 months home care experience.
• Current CPR/BLS certification.
• Empathy, reliability and a commitment to high-quality care.

Responsibilities

• Perform 1:1 patient care duties and treatments as per care plan.
• Provide health promotion, counseling, and education.
• Administer medications and other personalized interventions.
• Coordinate care, in collaboration with a wide array of health care professionals.
• Document nursing notes regarding patient care daily.
• Must be able to lift 50lbs.
• Must be able to stand/work for extended periods.
• Additional responsibilities as required.
