Job summary

Always Compassionate Health is seeking compassionate nurses to provide in-home care in New York. We offer flexible shifts and competitive hourly pay for LPNs and RNs, enabling you to make a meaningful impact one home at a time.

Active NYS LPN or RN license is required, along with at least 6 months of home care experience and CPR/BLS. Duties include G-tube/PEG care, tracheostomy care, medication administration, and collaboration with the care team.

Qualifications

• Active NYS LPN or RN license.
• Minimum of 6 months experience in home health care.
• Current CPR/BLS certification.
• Reliable, empathetic and patient-focused.

Responsibilities

• Provide 1:1 nursing care and treatments per the care plan.
• G-tube/PEG care, including feedings and meds.
• Tracheostomy care, suctioning, and ventilator support as ordered.
• Monitor patient condition and report changes to clinicians.
• Administer medications and document interventions daily.
• Coordinate with healthcare team and caregivers.
• Assist with activities of daily living and mobility as needed.
