Always Compassionate Health is seeking an LPN or RN to provide compassionate in-home care with flexible shifts. You will deliver one-on-one nursing, promote health education, and coordinate care with a multidisciplinary team.

Requirements include NYS licensure, at least 6 months of home-care experience, and CPR/BLS certification. Candidates must be able to lift 50 lbs and stand for extended periods; competitive pay is offered.

Formación

• Active LPN or RN license issued by NYS.
• Minimum of 6 months experience in a home care setting.
• Current CPR/BLS Certification.
• Empathy, reliability and a commitment to high‑quality care.

Responsabilidades

• Perform 1:1 patient care duties and treatments as per care plan.
• Provide health promotion, counseling, and education.
• Administer medications and other personalized interventions.
• Coordinate care with a broad team of health care professionals.
• Document nursing notes daily.
• Must be able to lift 50 lbs and stand for extended periods.
• Additional responsibilities as required.
