Always Compassionate Health in Suffolk County, NY, is seeking compassionate LPNs and RNs for 1:1 home care. Flexible shifts and competitive hourly pay ensure nurses can deliver high-quality care in patients' homes.

Qualifications

• Active LPN or RN license issued by New York State.
• Minimum 6 months of experience in a home care setting.
• Current BLS certification from AHAS.

Responsibilities

• Deliver direct patient care and treatments based on individualized care plans.
• Administer medications and tailored interventions.
• Promote wellness through education, counseling, and preventive care.
• Collaborate with multidisciplinary teams to coordinate care.
• Maintain accurate and timely nursing documentation.
• Assist with mobility and physical support (must be able to lift up to 50 lbs).
• Stay active and engaged during extended periods of standing or movement.
• Take on additional duties as needed to support patient well-being.
