Qualifications

• Active LPN or RN License issued by NYS.
• Minimum of 6 months experience in a home care setting.
• Current CPR/BLS Certification.
• Empathy, Reliability, and a commitment to high-quality care.

Responsibilities

• Perform 1:1 patient care duties and treatments as per care plan.
• Provide health promotion, counseling, and education.
• Administer medications and other personalized interventions.
• Coordinate care, in collaboration with a wide array of health care professionals.
• Document nursing notes regarding patient care daily.
