Responsibilities

Job Summary:

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the assigned geographic area. The nurse works under the supervision of a Registered Nurse or designated supervisor, following all applicable laws, licensure requirements, and agency policies.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within the scope of practice, in accordance with the care plan, and following infection control procedures.
• Documents patient visits thoroughly in the electronic medical record and utilizes electronic visit verification as applicable.
• Participates in care coordination, reports patient needs and changes, and communicates effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient information per agency policy.

Education and Experience

State Specific Requirements:

License Requirements: Must have current LPN licensure in Tennessee, a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Current CPR certification is required.

Experience Requirements: Previous home care experience preferred.
