Responsibilities

Job Summary: The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality service within the geographic area. Work is under the supervision of the Registered Nurse or designated supervisor, within the scope of practice defined by state laws and agency policies.

Specific Job Duties/Responsibilities

1. Provide high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
2. Document patient visits thoroughly in the electronic medical record, complying with regulatory and payer requirements.
3. Participate in care coordination, reporting patient needs and changes, attending case conferences, and communicating effectively with the healthcare team.
4. Maintain patient confidentiality per agency policy.

Education and Experience

License Requirements: Must have current LPN licensure in Tennessee, a current driver’s license, vehicle insurance, and access to reliable transportation. Current CPR certification is required.

Experience Requirements: Previous home care experience is preferred.
