Responsibilities

Job Summary:

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the geographic area. The nurse works under the supervision of a Registered Nurse or designated supervisor, adhering to state laws, licensure requirements, and agency policies.

Specific Job Duties/Responsibilities:

• Provide high-quality clinical services within the scope of practice, following the care plan and infection control procedures.
• Document patient visits thoroughly in the electronic medical record, complying with regulatory and payer requirements.
• Participate in care coordination, report patient needs and changes promptly, attend case conferences, and communicate effectively with patients, families, physicians, and team members.
• Maintain confidentiality of patient health information.

Education and Experience

License Requirements:

Current LPN licensure in Tennessee, a valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation, and current CPR certification.

Experience Requirements:

Previous home care experience preferred.
