Responsibilities

Job Summary: The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the assigned geographic area. She/he works under the supervision of the Registered Nurse and/or supervisor, adhering to state laws, licensure requirements, and agency policies.

Specific Duties:

• Provide high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
• Document patient visits thoroughly in the electronic medical record, complying with regulatory and payer requirements.
• Participate in care coordination, report patient needs and changes, attend case conferences, and communicate effectively with patients, families, physicians, and team members.
• Maintain confidentiality of patient information per agency policy.

Education and Experience

License Requirements: Current LPN licensure in Tennessee, valid driver’s license, vehicle insurance, access to dependable transportation, and current CPR certification.

Experience Requirements: Previous home care experience preferred.
