Job summary

Elara Caring is seeking a Private Duty Licensed Practical Nurse to provide in-home patient care, review care plans, and monitor patient status in Indiana. You will work with the RN Case Manager and clinical leadership to adjust interventions as patient needs change.

The role requires an active LPN license in Indiana (or NLC), 1+ year of nursing experience, and the ability to lift up to 50 pounds. A valid driver’s license or reliable transportation is required.

Qualifications

• Active LPN license in Indiana or Nursing Licensure Compact (NLC) scope.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1+ year of clinical nursing experience.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license or reliable transportation for travel between patient homes.

Responsibilities

• Review patient care plans, physician orders, and clinical information before providing care.
• Use clinical assessment skills to identify patient needs and make appropriate care decisions.
• Monitor patients for changes in condition and promptly communicate concerns to the physician and care team.
• Collaborate with the RN Case Manager and clinical leadership to determine interventions when patient needs change.
• Maintain accurate and timely documentation of care, patient condition, and changes in home or caregiver situation.
• Maintain scheduling flexibility to support changing patient care needs.
