Job summary
Elara Caring is seeking a Private Duty Licensed Practical Nurse to provide in-home care across Indiana. You will review plans, assess patient needs, monitor conditions, and coordinate with the RN Case Manager to adjust care as patient needs change.
The role requires an LPN license in Indiana, 1+ year of clinical nursing experience, ability to lift up to 50 lbs, and reliable transportation to travel between patient homes. Comprehensive benefits are offered.
Qualifications
- LPN license in Indiana or NLC-compliant required.
- Graduate of an accredited nursing program (Certificate/ Diploma/ Associate).
- At least 1 year of clinical nursing experience.
- Ability to lift up to 50 pounds and assist with transfers.
- Valid driver's license or reliable transportation to travel between homes.
Responsibilities
- Review patient care plans and physician orders before providing care.
- Use assessment skills to identify patient needs and guide care decisions.
- Monitor patient condition and promptly communicate concerns.
- Collaborate with RN Case Manager to adjust interventions as needed.
- Document care, patient status, and changes in home or caregiver situations.
- Maintain scheduling flexibility to meet changing patient needs.
