Job summary

Elara Caring is hiring a Private Duty Licensed Practical Nurse to support home-based patient care across Indiana. You will review care plans, assess patient needs, and work with the care team to adjust interventions as patient conditions evolve.

The role requires an LPN license in Indiana (or NLC rules), 1+ year of clinical nursing experience, and a valid driver’s license or reliable transportation to travel between patient homes. Comprehensive benefits are offered.

Qualifications

• Licensed Practical Nurse in Indiana (or NLC equivalent).
• 1+ year nursing experience in a clinical/home setting preferred.
• Ability to frequently lift up to 50 lbs and assist with transfers.
• Must have a valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Review patient care plans and physician orders before delivering care.
• Use clinical assessment to identify patient needs and inform care decisions.
• Monitor patient status and communicate changes to the care team.
• Collaborate with RN Case Manager to determine interventions when needs change.
• Maintain accurate documentation of care and patient condition.
