Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our Indiana home health team, delivering compassionate care in patients’ homes.

You will follow care plans, monitor progress, document medications, and coordinate with RNs and physicians to ensure patient safety and positive outcomes.

Requirements include an active LVN/LPN license, 1 year of clinical experience, ability to lift up to 50 pounds, and reliable transportation.

Qualifications

• State licensure as LVN/LPN.
• Graduate of an accredited nursing program (Certificate/Diploma/Associate).
• Minimum 1 year of clinical care experience.
• Ability to lift up to 50 pounds and transfer patients.
• Valid driver’s license or reliable transportation for travel between homes.

Responsibilities

• Follow each patient's care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with the RN Case Manager as needs change.
• Document medications and update profiles per guidelines.
• Communicate with physicians, patients, and families for coordinated care.
• Help prevent avoidable hospitalizations through proactive care.
• Deliver compassionate, high-quality care.
