Job summary

Elara Caring is seeking a Private Duty Licensed Practical Nurse to contribute to in-home patient care and collaborate with care teams. You will review plans, assess patient needs, monitor conditions, and document changes while maintaining flexibility to meet patient care requirements.

The role requires an LPN license, accredited nursing education, at least 1 year of clinical nursing experience, and reliable transportation or a valid driver's license to travel between patient homes.

Qualifications

• LPN license in the state or compliant with NLC rules.
• Graduate of an accredited nursing program (certificate, diploma, or associate).
• Minimum 1 year of nursing experience in a clinical setting.

Responsibilities

• Review patient care plans and physician orders before delivering care.
• Use clinical assessment skills to identify patient needs and make care decisions.
• Monitor patients and promptly communicate changes to the physician and care team.
• Collaborate with the RN Case Manager to determine interventions when needs change.
• Document care provided and patient status accurately and timely.
• Maintain scheduling flexibility to support changing patient care needs.
