Job summary

LiveWell Homecare Agency, LLC is seeking an LPN to provide nursing care in patients' homes, document clinical notes, and coordinate care with the healthcare team. You will report changes in condition and participate in case conferences while adapting to diverse home environments.

Requirements include current, unrestricted LPN licensure and a valid driver’s license; CPR certification is optional. Ability to work independently and communicate effectively is essential, with organizational skills

Job description

Role: Home and Community Based Services Licensed Practical Nurse

Responsibilities include: Providing direct nursing care to acutely and chronically ill patients in their homes, documenting clinical notes, and coordinating patient care activities to ensure high-quality service. The nurse collaborates with the healthcare team, reports patient needs and changes in condition, and participates in care coordination and case conferences. They must adapt to various home situations, regardless of patient background.

Qualifications: Current, unrestricted LPN licensure, a valid driver’s license or reliable transportation, and, optionally, CPR certification. Ability to work independently, manage multiple tasks, and communicate effectively. Preferred skills include organizational abilities and flexibility to meet patient needs.

Compensation & Benefits: Hourly pay ranges from $22.90 to $34.35, with additional benefits such as comprehensive health packages, incentives, and retirement options, based on eligibility.
