Job summary

LiveWell Homecare Agency, LLC is seeking a compassionate Home and Community Based Services Licensed Practical Nurse (LPN) to provide in-home nursing care to acutely and chronically ill patients in Greenville, TN. The role requires current LPN licensure, an active driver's license, and reliable transportation; CPR is preferred.

Responsibilities

Responsibilities: Provide direct nursing care to acutely and chronically ill patients in their homes, document clinical notes, coordinate patient care activities, and ensure high-quality services within the scope of practice. Participate in care coordination by reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, and healthcare team members. Ensure compliance with infection control procedures and regulatory documentation standards.

Qualifications & Skills:

• Current and unrestricted LPN licensure in the practice state
• Valid driver’s license, vehicle insurance, or access to reliable transportation
