Job summary

Open Arms Solutions in the Chicago area is seeking a compassionate Community Care Coordinator to help clients live independently and with dignity at home.

You will conduct in-depth assessments, coordinate care, support caregivers, manage a caseload, and ensure seamless communication between clients, families, and scheduling.

This full-time role requires 3–5 years in a high-touch client-facing setting, CNA preferred, ability to lift 50 lbs, valid driver’s license, and up to 50% travel.

Qualifications

  • 3–5 years in a high-touch, client-facing role, preferably in senior care, private duty, or hospitality.
  • CNA certification strongly preferred.
  • Proven experience in care management and leading a team of caregivers.
  • Strong communication, interpersonal, and problem-solving skills with discretion.
  • Knowledge of ADLs and IADLs for senior care clients.
  • Experience in private duty/home care service company.
  • Exceptional organizational skills and attention to detail.
  • Proactive, service-oriented mindset with a genuine passion for providing extraordinary care.
  • Proficiency in WellSky or other care management software and Microsoft Office 365.
  • Ability to lift up to 50 pounds.
  • Valid driver's license and reliable transportation.
  • Travel up to 50%.

Responsibilities

  • Support hiring decisions and assist in training and mentoring caregivers to meet concierge-level standards.
  • Match clients with caregivers based on skills, experience, personality, and lifestyle preferences.
  • Conduct regular quality assurance visits and check-ins to confirm plans are followed and clients are satisfied.
  • Anticipate evolving client needs and offer proactive solutions and personalized recommendations.
  • Act as liaison between clients, families, customer service, and scheduling teams for clear communication.
  • Maintain confidential records for clients and caregivers.
  • Participate in process improvements and system implementations as a super-user of cloud-based systems.

Open Arms Solutions is a premier private duty home care provider dedicated to helping seniors live independently and with dignity in the comfort of their own homes in Chicago area. We are looking for a compassionate and driven Community Care Coordinator to join our team and help us deliver exceptional care and support to our clients and caregivers.

Hours: Monday-Friday, Fulltime.

About the Role

The Care Coordinator is a key member of the Open Arms Solutions team and reports directly to the Executive Director. This role is responsible for ensuring exceptional service delivery for clients and supporting the professional growth and performance of caregivers throughout the West Suburbs, extending from Oak Brook, Elmhurst and other surrounding areas The ideal candidate is proactive, empathetic, highly organized, and experienced in the senior care service industry, with strong skills in client relations, care coordination, and caregiver support.

Responsibilities

  • Act as a dedicated liaison between clients, families, customer service, and scheduling teams to promote clear communication and timely resolution of issues.
  • Maintain accurate, thorough, and confidential records for both clients and caregivers.
  • Participate in process improvement, system implementation, and integration projects as a super-user of key cloud-based business systems.
Qualifications
  • 3-5 years of experience in a high-touch, client-facing role, preferably in senior care, private duty, or hospitality.
  • CNA certification is strongly preferred.
  • Proven experience in care management, managing complex client relationships, and supporting or leading a team of professional caregivers.
  • Strong communication, interpersonal, and problem-solving skills, with a high degree of discretion and professionalism.
  • Knowledge of ADLs and IADLs for senior care clients.
  • Experience working in a private duty/home care service company.
  • Exceptional organizational skills and attention to detail.
  • Proactive, service-oriented mindset with a genuine passion for providing extraordinary care.
  • Proficiency in WellSky or other care management software and Microsoft Office 365.
  • Ability to lift up to 50 pounds.
  • Valid driver's license and reliable, professional transportation.
  • Travel is required up to 50%.
