CareFor is seeking a compassionate and dedicated Registered Nurse On Call to join our team.

The RN On Call position will be remote, but will be responsible for driving to respond to after-hours needs for both Care Management and Hospice. Therefore, our RN On Call should live within the Georgetown to San Marcos area. We'll need our RN On Call to be available at short notice to make a visit if necessary.

As The RN On Call, You Will

  • provide timely and responsive nursing care to patients at all levels of service during after-hours, on weekends and holidays.
  • assess urgent needs, manage symptoms, provide crisis intervention, and ensure patient comfort in collaboration with interdisciplinary teams. This includes:
    • responding promptly to emergency and after-hours patient needs.
    • administering medications and treatments per the established plan of care or physician orders to manage pain and distressing symptoms.
  • coordinate care with families, caregivers, physicians, and other healthcare providers, ensuring seamless continuity of care between shifts.
  • assume responsibility for triaging patient concerns, making in-home visits as needed, and facilitating communication with the care team to maintain the highest quality of care. This includes:
    • providing phone triage as needed and assessing whether an in-person visit is required.
    • conducting immediate assessments of patients experiencing changes in condition, new symptoms, or urgent needs during on-call hours.
    • quickly evaluating patient and family concerns to determine the appropriate level of intervention.
    • Strong knowledge of nursing principles, techniques, and practices. Skilled in patient care, medication administration, and handling medical equipment. This includes but is not limited to wound care, G-tube feedings, trach care and more.
    • Ability to communicate clearly and effectively with clients, families, and healthcare professionals.
    • Ability to assess and address clinical issues in a timely manner, ensuring proper care for the client.
    • Demonstrated empathy and the ability to offer emotional support to clients and families.
    • Strong documentation skills and meticulous attention to detail to ensure accurate patient records.
    • Ability to adjust to a variety of client needs, care environments, and medical situations.
    • Understands and adheres to HIPAA regulations and always maintains the confidentiality of patient information
    • Specialized skills preferred in palliation and end-of-life.
