AbsoluteCARE Medical Center & Pharmacy is seeking an experienced Registered Nurse to support primary care activities alongside a dedicated PCP. You will educate patients, process orders and referrals, manage medications, and help close care gaps within the PCMH framework.

This role is on-site, Monday through Friday, 8-5 with an hour lunch, no weekends, no on-call, and includes paid holiday time off. Strong communication and organizational skills are essential to coordinate care with the

Qualifications

• Active, unrestricted RN license in state of practice.
• 3+ years of clinical experience with adult and geriatric patients with chronic conditions.
• Active CPR certification.
• Experience with community-based organizations preferred.
• Proficiency with Microsoft Office applications.
• Excellent interpersonal communication, problem solving, and organizational skills.

Responsabilités

• Act as the practice-based nurse, supporting on-site providers in facilitating high quality primary care.
• Facilitate PCMH functions through diagnostic order tracking, medication refills, chart review, and triage messages.
• Lead panel management by following disease care pathways and entering standing orders for preventive care.
• Coordinate with the care team to ensure timely follow-up and action plan completion.
• Provide medication reconciliation and patient education during in-person visits and telephonic follow-ups.
