Job summary

AbsoluteCare in Philadelphia is seeking a Registered Nurse to support PCPs with patient education, orders, referrals, and medication management within a team-based primary care setting.

Qualifications

• Active RN license in the applicable state required.
• 3+ years providing clinical services to adults/geriatrics with comorbidities.
• CPR certification required.

Responsibilities

• Support PCP panel activities and patient education.
• Coordinate diagnostic orders, refills, and care gaps with the care team.
• Facilitate PCMH functions and document in medical records.
• Assist with triage, wound care, IV therapy, and phlebotomy as needed.
• Assist with care coordination and patient follow-up after visits.
