Job summary

ProHealth Physicians, part of UnitedHealth Group, is seeking a full-time RN for our Primary Care team in Groton, CT. The role focuses on patient-centered nursing care, triage, and coordinating with providers in a busy outpatient setting.

Qualifications

• Active unrestricted RN licensure in CT.
• Experience in family practice or internal medicine preferred.
• Phone triage experience helpful.
• Experience with EMRs, preferably Epic.
• Strong knowledge of medical terminology.

Responsibilities

• Serve as liaison between patients, staff and providers.
• Document nursing care and patient interactions in the EHR.
• Prepare patients for visits by collecting vital signs and history.
• Perform injections and preventive screenings as directed.
• Assist care team to ensure smooth office operations.
• Provide patient education on disease management.
• Follow ProHealth policies and scheduling guidelines.
