Qualifications

• Registered nurse in good standing.
• BSN or ASN required/preferred.
• 1+ year of ambulatory care nursing preferred.
• New graduates and experienced nurses encouraged to apply.

Responsibilities

• Support primary care of patients under planned care model, including pre-visit planning, daily huddles, and nurse-led visits.
• Collaborates with PCP to identify preventive care needs and care coordination.
• Provides health education during visits and by telephone.
• Works with patients to identify self-management goals.
• Coordinates care with other community providers.
• Follows up on ER visits and hospital discharges.
• Assists with exams, treatments, and procedures.
• Administers medications including immunizations under orders.
• Documents medication administration and patient information accurately.
• Monitors patient response to medications and reports adverse reactions.
• Performs nursing activities such as wound care as needed.
• Documents all aspects of patient care in the medical chart timely.
• Schedules tests and referrals for specialty care.
• Maintains infection control standards.
• Acts as a resource for medical assistants.
• Oversees supply and equipment maintenance.
• Maintains mandatory annual training.
• Participates in care management projects and Integrated Care Team meetings.
