Qualifications

• Unrestricted CT license or ability to obtain before start date.
• Board Certification/Eligibility in Family Medicine or Internal Medicine.
• Active, unrestricted CSR and DEA or ability to obtain prior to start.

Responsibilities

• Provide outpatient primary care in a physician-led, team-based setting.
• Partner with Advanced Practice Clinicians and care teams to coordinate care.
• Educate and empower patients to take ownership of their health.
• Engage in growth, teaching, and leadership opportunities as desired.
• Support patients across multiple Connecticut primary care locations.
