Job summary

Hartford HealthCare Medical Group is seeking an RN to deliver skilled nursing care across multiple clinical settings under provider direction. You will assess, plan, intervene and evaluate patient responses using established protocols, reporting to a Practice Manager.

Responsibilities include collaboration with providers, triaging calls, and maintaining patient confidentiality while supporting family needs. Willingness to travel to regional offices as needed is expected.

Qualifications

• Current CT RN license and CPR certification on file.
• Three years of experience in a medical practice preferred.
• Experience with medical software applications.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses patients’ physical and psychosocial needs; plans and implements care.
• Demonstrates independent nursing actions based on assessment.
• Triages calls and initiates appropriate nursing interventions promptly.
• Assists patients and families with empathy; maintains confidentiality; travel to regional offices as needed.
