Hartford HealthCare Medical Group is seeking an RN to provide skilled nursing care under provider direction across multiple clinical settings in Connecticut. This role emphasizes nursing assessment, care planning, and adherence to standards and guidelines.

The position requires an active Connecticut RN license and CPR certification, with preferred experience in a medical practice and familiarity with medical software. Travel to regional offices may be required as part of this role.

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner.
• Travel to other regional offices, as needed.
