Qualifications

• Current state of Connecticut RN license and current CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provide skilled nursing care to patients in various clinical settings.
• Assess physical and psychosocial needs of patients and plan care.
• Demonstrate independent nursing actions based on assessment.
• Triage telephone calls and initiate appropriate nursing interventions.
• Assist patients and families with concerns while maintaining confidentiality.
• Travel to other regional offices as needed.
