Job summary

Hartford HealthCare Medical Group is seeking an experienced RN to provide skilled nursing care under provider direction across multiple clinical settings. The role reports to a Practice Manager and may travel to regional offices as needed.

Candidates must hold a Connecticut RN license with CPR certification and have 3 years of medical practice experience; proficiency with EMR software and strong patient communication are valued assets.

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner.
• Travel to other regional offices, as needed.
